Our commitment to your privacy
NueVida Oncology, a division of NueVida Physician Group, is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment. We use your information to provide, coordinate, and manage your care — including sharing it with other physicians, laboratories, pharmacies, and facilities involved in your treatment, and with NueVida Physician Group colleagues who participate in your care.
- Payment. We may use and disclose your information to bill and collect payment for services, including verifying insurance coverage and obtaining prior authorization.
- Health care operations. We may use your information for quality improvement, training, licensing, and other activities necessary to run our practice.
- Appointment reminders and care communications. We may contact you by phone, text message, patient portal, or mail about appointments, results, and treatment options.
- As required by law. Including public health reporting, cancer registries, health oversight activities, and responses to lawful requests.
Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time in writing.
Your rights
- Request restrictions on certain uses and disclosures of your information.
- Request confidential communications by alternative means or locations.
- Inspect and obtain a copy of your medical record (see our medical records request process).
- Request an amendment to your health information.
- Receive an accounting of certain disclosures we have made.
- Obtain a paper copy of this notice on request.
- Be notified following a breach of unsecured PHI.
Changes to this notice
We reserve the right to change this notice and make the revised notice effective for information we already have as well as information we receive in the future. The current notice is posted in our office and on this website.
